PALMYRA, Pa. — The driver who pleaded guilty to hitting a Palmyra woman and leaving the scene of the deadly accident, was released from prison on a clerical error. Chad Ryan Collins, 30, is back in police custody after turning himself in to Palmyra Police this morning.
Collins struck and killed Lisa Thomas on E. Cherry St. in Palmyra on the night of Oct. 21, 2015. Collins had initially stopped but then fled the scene. Thomas died later of her injuries. He posted $50,000 bail and was released from Lebanon County Prison on January 8, 2016. He was then arrested in Lancaster on January 26th. Because of that arrest, Lebanon County Court of Common Pleas Judge John Tylwalk revoked his bail and reset it at $100,000.
Collins pleaded guilty to the charges involving Thomas death in July, while he was in Lancaster County Prison. He pleaded guilty to his Lancaster County charges in August and was sentenced to probation. Collins was released, with Lancaster County officials not being aware of his revoked bail status from Lebanon County.

He was released from Lancaster County Prison on August 22nd. Collins turned himself in to Palmyra Police at 8:42am.
